Verify Benefits

Within 24 hours, we will verify the patient's coverage and schedule an assessment within 72 hours. If a visiting nurse is involved, we’ll coordinate with the home health agency for seamless care.

Patient Assessment

We’ll assess the wound, develop a personalized treatment plan, and educate the patient or family on proper care. We’ll also share the plan with the visiting nurse to ensure smooth, continuous care.

Weekly Ongoing Care

We’ll provide weekly visits to treat the wound and monitor progress, while sharing chart notes with the home health agency to ensure everyone stays informed on the patient’s healing journey.
